Please find below a list of all forms that each parent/guardian/player must fill in.

1. Codes of Conduct
Read this one very carefully as this is the way we want all our players, parents, spectators and managers to act throughout the season. Sign the last page and cut the bottom part off and return to the relevant Secretary, you keep the main part for future reference.

2. Club Membership & Parental Consent Form
Fill in all the information asked for, it is required for registration and communication purposes. This form also gives the manager/coach permission to coach and manage your child on training, match and other activities.

3. Personal Disclosure Form
This form is vital, if the manager/parent/guardian are taking children to and from training, match's and any activity Penwortham Town F.C. are organising, any manager/parent/guardian not filling in and Signing this form will not be permitted to attend training, match's and any Penwortham Town FC activity.

5. Image Permission Form
This form gives the club permission to use images of an under 18 on the web site inline with the policy set out by the FA.(A copy of which can be found under Policies or by clicking here). If you do not wish to sign this form it is not compulsory but the child would be excluded from Club Photos etc. for the website.

6. League Registration Form (not on website)
This form goes to the relevant league your child plays in, again fill in all the required spaces, if you don't the league could refuse your child's application.

Penwortham Town FC. understand the problems of filling in all the forms, but new rules and regulations are coming from The FA. all the time and we have to comply with them. If you study the forms you will realise the importance of them, in all it will take
about 1 hour to fill them all in, and your child will have a whole season of football and other activities.
